What I need from you for a quote:
1) Name of Contact Person and phone numbers
2) Pick-up address (city/state)
3) Drop off address
4) What are your Time Constraints
5) Health Concerns
6) Number of Pets and size
7) Breed and Age of Pet
8) Health certificate valid for duration of transport
9) Rabies certificate (pets 3 months or older)
- Including Distemper and Bordetella also required
NOTE: If giving vaccines for the first time it must be done no later than 3 days before the date of travel

Payment Options:

I invoice through QuickBooks with the option for payment either through a bank transfer (no fee) or with a credit card (4% service fee to be added). Payment must be made at time of bookinig. I will need customers e-mail address to send invoice.

Owners are not allowed to accompany their pets. Owners are responsible for any damage the pet produces.
A grooming fee will be applied if your pet has an accident that requires grooming during transportation.
Coast2Coast Pet Transport LLC reserves the right to refuse transportation to any pet that cannot be transported safely.
